چکیده

EHUD I. ASSIA, MD The combination of advanced glaucoma and phacodonesis with lens subluxation suggests pseudoexfoliation (PXF) syndrome, which needs to be ruled out. Another potential cause is high myopia, in which significant phacodonesis is not uncommon. Cataract extraction is certainly indicated, not only to remove the opaque media but also because of the patient’s glaucoma. Anterior displacement of the unstable lens may play a role in the high IOP, and removal of the crystalline lens is often associated with decreased pressure. Intracapsular cataract extraction and the implantation of either an ACIOL (angle or iris supported) or a suturefixated PCIOL are valid options. My preference, however, would be to preserve the posterior capsule by fixating the lens capsule to the scleral wall. The Cionni Rings for Sclera Fixation and the Ahmed Capsular Tension Segments (CTS) (both from Morcher GmbH, Stuttgart, Germany; distributed in the United States by FCI Ophthalmics, Marshfield Hills, MA) were shown to be highly effective for fixation of the capsular bag to the scleral wall. I have developed with Hanita Lenses (Kibbutz Hanita, Israel) an alternative device, the AssiAnchor (not available in the United States), which clips to the anterior capsule Phacodonesis and a Subluxated Crystalline Lens

Comparison of Anesthesia with Sevoflurane and Propofol on Extubation time and Recovery Parameters During Cataract Surgery

Background: One of the most common outpatient surgeries is cataract surgery which requires a short anesthesia with fast recovery time providing minimal complications. This study aimed to compare sevoflurane with propofol in terms of extubation time and recovery parameters in patients undergoing cataract surgery. Materials and Methods: This randomized controlled clinical trial recruited 60 ...

Management of Traumatic Cataract with Posterior Capsular Rupture: A Case Report and In Vitro Model Study

PURPOSE To investigate the optimal strategy for surgical management of traumatic cataract with posterior capsular rupture. METHODS We describe four cases of traumatic cataract with posterior capsular rupture and an in vitro model built to evaluate the optimal infusion pressure during surgery. RESULTS All patients underwent cataract surgery. By using an anterior chamber maintainer to elevate...

Phacoemulsification: Considerations for Astigmatism Management

Patient expectations for cataract surgery have increased significantly over the past several decades as surgeons have transitioned from standard extracapsular techniques to clear-cornea phacoemulsification. In addition to short operating times, topical anesthesia, and a sutureless procedure, patients increasingly expect spectacle independence following cataract extraction. In this tutorial we p...

Light-adjustable intraocular lens in post-LASIK and post-traumatic cataract patient.

The case illustrates the use of a light-adjustable intraocular lens (IOL) in a patient who had had refractive surgery and developed blunt trauma with resultant flap damage and traumatic cataract. Consistent with early reports of the light adjustable IOL, which show positive results in non-LASIK eyes, this case suggests that the light-adjustable IOL may have similar benefits in the post-LASIK ey...

The effect of low dose of ketamine on intraocular pressure in patients undergoing cataract surgery

Background: Cataract surgery is one of the most common surgeries in the world, especially in elderly patients, and often performed topically with sedative agents. Ketamine is one of the most commonly used agents and the effect of different doses on intraocular pressure is in dispute. The present study investigates the effect of a low dose of ketamine on intraocular pressure in patients undergoi...

Do patients with age related maculopathy and cataract benefit from cataract surgery?

AIMS To assess the benefits of cataract extraction in patients with age related maculopathy (ARM). METHODS 1073 randomly selected cataract operations were reviewed and 99 cases of preoperatively recognised ARM were identified for investigation. Data relating to visual function were retrieved from case notes, and patient responses to a questionnaire were analysed. RESULTS 98% had dry or unsp...
